I don’t understand what you mean by grade either, but there is no reason to do a biopsy to diagnose BPH. What really matters is how much the symptoms bother you and treatment is not always easy. Be sure to see a urologist for this do not rely on your primary care physician.

I’m not sure what you mean by "grade". BPH is diagnosed basically the same way prostate cancer is, starting with a digital examination of the prostate via the rectum and followed with ultrasound and biopsy. It is a very treatable condition and I would trade my cancer for it if given that choice!
